Falls and Telecare Assessment program serves as an umbrella for ongoing fall preventions. We work with individuals who are at risk of falls or have a history of falls. Falls are a significant concern, especially for older adults and individuals with certain medical conditions or mobility issues.
Falls and Telecare Assessment
We use fall assessments to identify factors that contribute to the risk of falls and to develop interventions to mitigate these risks. These might include:

Remote Monitoring
Wearable devices or sensors can be used to monitor an individual’s movement patterns, gait, and activity levels. Data collected from these devices can provide insights into changes that might indicate an increased fall risk.
Virtual Consultations
We will conduct assessments, provide education, and offer guidance through virtual appointments. This is especially useful for individuals who may have difficulty traveling to in-person appointments.
Home Safety Recommendations
We can use telecare to virtually assess an individual’s home environment and provide recommendations to modify the home for improved safety. these may be sensors that detect movement, door opening, room temperature, smoke/gas appliances that have been left on. these sensors will send alerts to your family, carer or a monitoring service.
Rehabilitation Programs
Telecare platforms can deliver customised rehabilitation programs that focus on improving strength, balance, and mobility. Therapists can guide individuals through these programs remotely.
Education and Training
Telecare allows therapists to educate individuals and their caregivers about fall prevention strategies, proper body mechanics, and how to respond in case of a fall.
